&
NJ STATE GUIDELINES

    ​COVID-19 pandemic - client disclosure

    ​This client disclosure form seeks information from you that we must consider before making service decisions in the circumstance of the COVID-19 virus.
     
    A weak or compromised immune system (including, but not limited to, conditions like diabetes, asthma, COPD, cancer treatment, radiation, chemotherapy, and any prior or current disease or medical condition), can put you at greater risk for contracting COVID-19. Please disclose any condition that compromises your immune system & understand that we may ask you to consider rescheduling your service after discussing any such conditions with us.
     
    It is important that you disclose to Signature Barber any indication of having been exposed to COVID-19, or whether you have experienced any signs or symptoms associated with the COVID-19 virus. Thank you.
    ​I certify that I DO NOT have a runny nose, sore throat, dry cough, fever, or above normal temperature; and in the last 14 days, I HAVE NOT experienced shortness of breath, had trouble breathing, had a reduction in my sense of smell, or traveled outside of the U.S. by air or cruise ship. 
    I fully understand and acknowledge the above information, risks and cautions regarding a compromised immune system and have disclosed to my provider any conditions in my health history which may result in a compromised immune system.

    By electronically signing & submitting this document, I acknowledge that the answers I have provided above are true and accurate.

  • Any individual who answers “YES” to any one of the screening questions is advised that the appointment cannot take place and if they arrive at the premises, they will be sent home and not permitted to proceed with the appointment, without being charged for the appointment or a cancellation fee.
  • If the client responds in the affirmative, the client is to be advised that no appointment can be scheduled for at least 72 hours after being symptom free, without having taken fever-reducing medication or before 14 days after exposure to a suspected or confirmed COVID-19 positive individual.

Guidelines Implemented for Safety at Signature Barber​
​

  • All staff has been trained in infection control procedures, cleaning & disinfection, proper use of PPE, etc.
  • Along with your booking, you can now pay online for a contactless payment of your service. 
  • Prior to your scheduled service, you will receive an appointment reminder.
  • A no-contact forehead temperature check will be administered. Temperature should not exceed 100.4 degrees, along with individuals accompanying a child. (Staff must undergo same procedure)
  • Cloth face covering/ masks are optional.
